Idle Woes: 765T 700 1990

Folks,

1990 765T with 218k miles.

Warm idle with transmission in park: 1100rpm
Warm idle with transmission in drive, brake engaged: 600rpm

Cold starts suck - requires accelerator.

Recently work:

New IAC, new temp sensors, new alt, new battery, cleaned TB, no exhaust leaks, new tranny with new kickdown cable.

I'm afraid of this system. The dealer "fixed" my idle problem with the new IAC and cleaning a plugged crankcase ventilation system, but it returned no more than a week later. Why would I have great idle with the transmission engaged, but HIGH when in park (warm) and LOW when in park (cold)?
