Manual transmission shifting problems 700 1987

I have an '87 Volvo 740 turbo intercooled (though I'm sure the turbo intercooled doesn't apply to the problem). First I bought the car from an estate - no warranty, I was told though the transmission had recently been rebuilt. The problem is this: you can feel a distict rattling/tapping sound when the transmission is in neutral. It reluctantly goes into first and down the road you go. If you lean your hand on the shifter after going into second, it will make a real bad rattling sound (should I call it grinding?), the same from 2nd to 3rd, and 3rd to 4th. The transmission will not go into overdrive from 4th. It is very difficult to get into reverse, you really have to push the shifter hard over to catch reverse. Is this all related to linkage adjustment? Really would appreciate where to start to correct the problem. I am mechanically inclined just lack the insite where to start
