Volvo 240 Rear End/ Trany 200 1986

I am having a peculiar problem. It's rather beguiling to me. What happens is when I break hard, or if the car is cold and I come to a stop, the transmission slips out of gear. Then when I need to go again, it winds up for half a second, then it catches into gear and goes with a bang. Like I say, it happens if I come to a Hard stop. Also it happens when the car is very cold. I didn't notice it this summer, but perhaps that's because it was warm out, and not it's very cold. It always happens at the first stop I come to. Sometimes the second, and possibly the third also. Then once the car is warm it doesn't happen as much. One thing that I have has since I have had the is a clunk in the rear end. I don't think it's a transmission problem, more of a rear end problem. What do you all have to think?
