replacement of inner and outer tie rods... 200 1984

drove home from milwaukee to indianapolis yesterday and the car made it, but i took it about 45 all the way. Above 45 the cars front end would shake and make audible clanking noises, the boot on the drivers side inner tie rod/ball joint? is torn and inside is some rust. my assumption is that this is the biggest problem. im pretty sure both tie rod ends need to be replaced as they are very old (previous owner was very negligent) and look bad. how difficult is it to replace tie rod ends and the other associated bits? do i need any special tools? also, what is perhaps a test to see exactly what is wrong? any help would be greatly appreciated.
