New clutch slipping? 200

Ok, about two months ago I put in a new clutch on my '85 240 wagon. Been working great, nice a smooth, no problems at all. About a week ago I was driving on the freeway in 4th and went to go into 5th (OD). I use the clutch for going into OD (don't know if it's best to do that or just to push the button and let it go into gear by itself w/o clutch). Right before I pushed in on the clutch I thought I heard the clutch slip because the engine revved to about 5k rpms. I brushed it off as maybe I started pushing in on the clutch and didn't realize it. But today I started out at a light in 1st and once the clutch pedal was all the way out the clutch was slipping A LOT! Took about 3-4 seconds before the clutch finally grabbed. Hasn't done it since. Does this sound like I just need to adjust the clutch a little? The clutch pedal feels fine with nothing else wrong. Am I missing something? Thanks.
