Starting problem with Volvo 240

I have a 1986 Volvo 240Dl with the B230 engine and the problem I have is as follows: The car starts easily but it immediatly dies. It starts easily again and I let it warm up some but as soon as I put the car in gear the engine stumbles and dies. When I start the car for the third time it runs fine and I don't have any further problems. This problem as been consistenly going on now for a few months. I had it in the shop and they adjusted the air mass meter which didn't help. They also cleaned the valve which controls the fast idle speed. I didn't see any improvement after that either. Also I have noticed some black smoke comming out of the tail pipe on initial start up and the guys in the shop said it looked like the plugs had a reach mixture. If anyone has experienced a similar problem or has some helpful suggestions let me know. Thanks.
