Just how tight should the oil drain plug be 200 1988

I have changed my own oil for thirtyfive years, no problems. Until my last oil change. I tightened it exactly as I always did, and was driving to work one day and tha oil light came on!! My drain plug fell about 100-200 yards back. But I shut her down immediately, and it didn't hurt the engine. Still not sure why it fell out, but I have never changed out the copper washer on this car.
