Air Conditioner Safety Valve 700 1989

It appears my AC/134 has vented through the safety valve. Is there a likely culprit or just the valve is bad? I had a mechanic replace most of the AC system several years ago and after that it took several tests/refills to determine that the safety valve needed to be replaced.

Now it is venting at the same spot. It looks like the actual valve - not the pipe - is made in japan. The muffler pipe was replaced with a new volvo pipe.
