Failed Annual Inspection 900

My '92 940 just failed its emissions test - high NOx (std was 910, reading was 1648). Last year had a similar problem and a new converter and tune up did the trick. That was about 20K miles ago. Where would you suggest I start. Is it likely the converter is bad this quickly?
