I've never seen one of those washers on any of my engines, including a couple that I doubt had ever been apart before (like my 1800E's motor - I bought the car with 72K miles, and had to replace the cam after about 15K miles).

As for locite - very good idea. The bad lifter and new cam my PV's motor recently needed was the result of the nut loosening and allowing the cam to slide backwards, until the lifters were running half off the lobes. It really didn't make many unusual noises while doing this either.

I've seen wear on brass ones before, the back of some new cam gears sometimes is not very smooth. Odd for what is a bearing surface - even if it doesn't get much load.
