What's involved in replacing the heater vent control vacuum hose? 200 1980

I'm talking about this hose here on my '80 245 DL:





As it is right now, the hose is hard, brittle and developing cracks. Every few weeks I end up having to cut the end off of it because a large split seems to develop where it attaches to the intake manifold, but I'm starting to run out of slack.

Just how much trouble is it to replace this hose coming out of the firewall? Where does it attach inside?
