Noisy engine on occasion 700 1988

My brick will occasionally, at low rpms, and low speed
produce blood curdling sounds from the top of the engine.
It sounds as though something is faulty in the valve train area.
Some times it'll be fine for days. Then, it comes back for few
minutes, only to disappear again.
If this was a minor noise, piston slap, or a mild ticking, I wouldn't
care. But, it really sounds bad. Drivers around me are taking notice.
What could it be ?
