Loose Brake Pads 700 1990

For at least six months I had a front end rattle and did not have the money for new bushing so I just lived with it. But yesterday when I did my rear brakes I decided to take off the front wheels and take a look. Well what did I find? That though my bushing were looking old and worn I had a loose inside brake pad. The little spring was not holding it in. It was just sitting there rattling around. My braking had always been good and the pads are still very new I did them about six month ago. Of course I putt the pad back in. What could cause such a thing? I am convinced that I put the pads in correctly the first time.
