Sporadic hard to start 200 1990

The cold start injector is only used when the engine is cold and the ambient temp. is below @ 20' F. How long between start-ups does she sit? Check the coolant temp. sender on rear of driver side under the intake manifold. When the engine is cold the temp. sensor sends a signal to the brain which sends signals to the CIS and MAF to enrichen the fuel mixture until engine temp. reaches normal operating range.

I would think if it is the fuel reg. it would sputter when you step on the gas; not the other way around.

Here are some other possibilites although I doubt it because yoou said onluy at start-up.
It could be a vaccuum leak (intake man. gasket, throttle body, CIS hoses, vaccuum hoses,exhaust man. gasket and doughnut, leaking injectors, Throttle switch, Constant Idle Motor (CIS)

What ever the problem is it seems to be a lean fuel mixture at start-up.
Have you run the diagnostic on it(located on driver's fenderwell?) That should tell you where to start looking.
