1989 240 clunk from steering plus strange transmission 200

Hi everyone,
I have just bought an 89 240 wagon and I have noticed a fairly loud clunk coming from the steering when turning the wheel either to the left or right.Mechanic thought it was loose crossmember bolts and he tightened them.The clunk is still there.Car had a reconditioned steering rack put in about 50 000kms ago, and now has 215 000kms.
In addition to this when you shift the gear lever into drive there is sometimes a short time lag before it actually shifts into drive.There are no other probs with the transmission and it shifts smoothly.
Any ideas anyone?
