Help to diagnose grinding noise from 1st to 2nd gear

I have a 1992 745. There is a grinding noise and a hard jerk when it shifts from 1st to 2nd gear. Remaining shifts are pretty smooth. I just flushed the AT fluid using the coolant return line method. The shift from 1st to 2nd gear is smooth if I am soft on the gas pedal and accelerate slowly and make sure engine RPM is steady around 2500.

Please let me know what my next step should be. Thanks.

Just an addition, this hard shifts happens only occasionally and NOT all the times. Thanks again.
