My understanding is that the usual FPR failure mode is that the diaphragm leaks, allowing fuel to be sucked into the manifold via the vacuum line that normally runs the fpr. This gives the rich mixture. The air/fuel mixture management is measuring the air intake and metering fuel - but considerable extra fuel is being added via the leaky fpr which is not being metered at all.
I suppose it's also possible for the fpr to fail in not bleeding off excess pressure into the return line towards the fuel tank. In which case the injectors would receive excessive pressure.
