Make sure it's not going in cross threaded.
I once put #1 plug in at an angle. Not smart.
The only thing I can suggest is, with plug started in the threads, eyeball it comparing to the other three. Should be parallel. If not, then you cross threaded it the first time you started it going in and have been following those new wrong threads every time.
