Joe,

Your clutch is the problem, period.

Springs on clutch disc/plate ware out, so even if you think you have a fully adjusted fork-play and all - the clutch manage to spin the in-shaft in the gearbox.

You'll need a complete clutch exchange -pressure plate, disc and release bearing. (Remember to buy a set!).
