Ball pein hammer on an exhaust manifold or running like a Diesel, or sound of rod knock is the timing gear - my bet. I'd get out your stethescope or use the screwdriver to the ear trick and see if that is where the trouble is for sure. If you've already got a replacement and some suspission that it was going to die, take the cover off and have a look. I'm sure the fiber gear will fall off in your hand.
