You have gone thru just about it all--I don't think it's the temp sensor
because the engine idle's well after warming up--by that I mean that you don't have a high idle around 1200 rpm--my best guess is that you have a fuel injector
going bad--my best guess--lots of times, ya just gotta be there, ya know
You might have some luck trying to isolate the cylinder thats giving the problem by disconnecting the coil packs one at a time and seeing if this changes the problem--heck maybe its a coil pac and if you find the cylinder then trade some coils around and see if it's a coil causing the problem---main thing is to first isolate the cylinder thats causing the problem----if you can unhook the coils to find which cylinder is screwing around with you and you trade the coils and if it doesn't solve it --try trading the power stages between the banks and see if the cylinder changes then---there are a number of things to look to before starting to spend hard earned dough ray me.
