Could be an air leak arround the intake gasket. Gets worse when the car is warmed up. If you think this may be the case you can test it with a propane bottle with a torch end. With the engine running, point the torch end arround each injector and leave it there for at leas 10 seconds, move to the next. If you feel the idle changes to better, then there is a crack in the gasket. Common thing around the 200,000 mile marker.
