The "rust ring" I described in my original post was real, but from an earlier occasion. The one I was really referring to had the rust "void" in a different area of the inner surface, as shown below. Actually, I've seen the vibration caused by both inner and outer rust voids.
The pictured one is only about .010" deep, but when it was gone so was the braking shudder.
