Check the radiator...outside to see if its clear and not stuffed with dead bugs. Try a radiator flush. Maybe some of the interior tubes are clogged. If the radiator is beyond cleaning, it can be re-cored. You could also try a new gasket on the radiator fill cap and the overflow bottle cap. If you are losing coolant slowly, maybe the system isnt coming up to proper pressure.
For the fan clutch, with the engine off, try to spin the fan. It shouldnt turn more than about 1/4 turn....maybe a little more... before stopping. Also, wiggle the fan back and forth; front to back. It should have almost no play. If the fan turns easily or it wobbles, then the fan clutch is probably worn. However, you say the temp rises even as you drive, which is when the fan becomes less important and just air blowing over the radiator should be adequate for cooling.
