You tried adjusting the reverse gate inside the upper shifter (in the cabin)?

That is likely the cause, since you say that 1 and 2 are both a problem, and the rest aren't. Also you don't mention any noise either.

The 5-speed is a good trans, but it is NOT a good trans if you don't keep fresh fluid in it. Overfill it (tilt the car on the right) and it'll stay happy for a long time. Type F fluid is good, as is Redline MTL. And Amsoil Type F synthetic.
