It's a regular sealed beam. 89 is the last year for them. You can get them at any auto parts store. Recommend you replace them in pairs- the difference in resistance between an old lamp and a new one is often enough to turn on the "Bulb Out" indicator. They're cheap enough and you only need a phillips screwdriver to replace- I always loosen the bottom screws on the frame, then remove the top ones and sort of bend the surround outward till the bulb comes free.
