Can't answer your "common causes of dash light failure" but I just recently replaced the bulbs for my instrument cluster (tach, fuel etc). There's a total of 4 3W bulbs in the back. Unscrew the bulb-holders and just pull the bulb out of the holder. 2 out 4 in mine was kaput but as a preventative step I replaced all 4. BTW much easier to take it out as in the FAQ. Didn't even had to remove the wires.
