Worn ignition switch sounds like the culprit. If you wiggle the key around, see if that helps any in creating or eliminating the problem.

iPd sells replacement switches. Not hard work - just a bear to get to. Pull the connector, and two screws removes the switch off the back of the lock.

Defogger might be because of worn wires at the hinges, shorting with one another.
