I assume we're talking an automatic tranny here. There is a section in the FAQ here:

That may be your problem.

I had a Dodge automatic lose reverse once and it was a broken part inside the tranny. I got a tranny shop to replace that part and all friction discs for about $600. I think I got a pretty good deal.
