You didn't mention anything about the idle air control valve. Test it as per Bentley manual. One other thing to check is the throttle position switch, located just below the throttle body. It's a plain switch and an ohmmeter is all you need for to test it. When the switch is 'off' (when the gas pedal is applied), the air flows through the throttle body (or is controlled), but the switch is in 'on' position (or when starting the car) the idle air control valve takes over.
Vacuum leaks are something to search for as well.
