The "two plastic gears with a hex type hole in the centre" are for left/right adjustment. I've aimed my right headlamp slightly to the right of centre in order to throw more light on the edge of the road. The "white nylon nut with a cross philips type screw" behind the lamp housing adjusts up and down. This one will get your level bubbles back on centre, but as the other poster mentioned, these things sometimes loose their accuracy after a while.
