I don't think it's a part issue at all. It's a weak design that caused the problem (in my opinion).

Pressure for short term profitability can create Shortened development cycles and subsequent cutbacks in testing and quality control. That's what can allow poor concepts to make it to the market. This kind of environment often takes hold in a corporate takeover environment.

Seriously, how could an engineer approve the design of a heater blower that can ever POSSIBLY freeze. I can image a marketing executive push a design through regardless just to get it to market.
