When I picked up my car I spoke with the service manager. He didn't have any ideas as to why mine failed. He did say that this was a FIRST for them. This was the first bevel gear the dealership (in a town of 100k+ population) has replaced. He did say that Volvo gets the core back and tears them down to see what happened.
