You may be seeing the effect of a failing fuel pump. They get mixed reviews here, but a search showed at least one that went 229K miles, while some go bad at 150K or sooner. I replaced mine (preemptively) at 157K.
The 2-2-1 code could also be related, due to intermittent low pressure and lean mixture during one of the pumps more severe "bad spells".
Fuel pressure might read OK at Idle—using all the fuel delivered but returning little or nothing to the tank. But you could put an ammeter in line with the pump feed wire (pink) at a connector in the left rear, behind the side panel. I'd expect at least 7.0 amps, based on what my replacement pump reads.
