The usual question asked is "have you cleaned the throttle body?" Since you don't mention having done so, that would be the place to start. The problem arises when the throttle position switch (TPS) can no longer close completely at rest or the idle motor gets gummed up with condensed crankcase vapors. Cleaning the throttle body goes hand in hand with cleaning the idle air valve and checking the TPS operation.
