Start by checking the usual... clutch operation, adjustment, etc.

Of course you did say 2nd and 4th, clutch stuff is usually worse in 1st and reverse.

Double clutching should not matter here, but... my gut feeling says transmission mount.

187k is seemingly low for a M46 to go, but certainly not unheard of.

If it is syncros, you can double clutch 'em forever.. as long as you don't grind gears.

But check that transmission mount.
