Ejected Spark Plug 700

Had some issues with a spark plug being loose the other day, and I re0tightened it.

Today, I tried a warm restart, and heard a very loud misfire - I opened the hood and found my frontmost spark plug ejected. I put it back in, with a lot of difficulty with the threads, and drove it a mile home.

Any ideas? I've been suggested to get a thread chaser. I haven't had a chance yet.

Has anyone heard of this happening before?

Any help is appreciated,
