oxygen sensor, check engine light 200 91

My volvo is setting codes 221,232,and 231 which the Volvo Techtips says are Heated Oxygen Sensor not operating, Self Adjusting Oxygen Sensor not operating properly(idle) and Self Adjusting Oxygen sensor not operating properly (part load). They suggest checking fuel pressure, Mass Air and for intake leaks. What is the chance that the Oxygen sensor is no good and were is the connection to it and is it hard to replace.?
