One very likely possibility not mentioned is a very dirty throttle body. If it's gunked up, it may prevent the throttle from closing 100%, and not contacting the throttle position switch.

Without TPS closure, the injection system does not go into "idle" mode and the idle will hunt up and down, coming close to stalling even when warmed up.

The gasket for the throttle is like 79 cents- buy one and a can of carb cleaner, take it off and clean it up.
