Lug nuts were loose, now swaying in rear? 200 1988

Took a two 1/2 hour ride this weekend w/ my 245 fully loaded (4 adults, 2 kids, a couple suitcases). Halfway thru the trip I thought I heard a noise, everyone else said no. When we got to our destination it became apparent there was a problem. I checked & there was loose lug nuts on one rear wheel. My 14 year old put my snow tires on this year & I thought I double checked for tightness but maybe not. Two 1/2 hour ride home was fine BUT : now my wife & kid say there is "swaying" in the rear especially when just starting out after stopping. Tire damage? (I don't see any). Rear axle bearing? Any suggestions appreciated.
