Well let's see,

Youve got spark!
We can assume you have good compression as it run's well normally at higher then idle!

I would start to think here you might have a clogged injector.

In your second post you said all 4 plug's looked good, so you cannot be sure at this point the #4 is not fireing it might be and this could be something as simple as checking your ignition part's and sensor's, get in there and clean out your idle control motor if its shot you wouldnt get enough air at idle but you could get just enough to hold an idle. Id rule out the fuel and more look into the ignition part's, remmeber you said all 4 plug's look good, so your getting fuel ( at least enough to idle ) and your getting enough air with the fuel check those electronic part's.
