First, you check the integrity of the FPR diaphragm by pulling the VACUUM hose, not the return hose.

Digital meter are almost worthless when checking an O2 sensor, unless you are using something with a fast refresh rate (>5x/sec). Analogue meter is much better for this.

You are getting two codes for bad O2 sensor and you said the output voltage is not right (without being specific what that means). Maybe the sensor is contaminated beyond recovery.
