yeah, I'm with Kombi. No way would I trust my wheel (life) to a nylock - although we often use nylocks on the steering parts...

Are you sure you're looking at the right hole? last time I worked on my hubs and was sure that something was wrong. It turned out that the cotter pin hole was not where I thought.

I think I've experienced what you are describing. In fiddling with the setup, I was always able to get the wheel hub seated so that everything lined up properly. It was not just an issue of not torquing up the nut properly, it has something to do with how the bearings were seating themselves on the spindle. I never really found out what was wrong, but I was always able to correct the issue.

It may be an pocket of air or something, sorry I can't be more informative.
