Pull the cover off the timing gear and examine the large fiber gear for a split, teeth partially missing, or a separation of fiber gear from brass hub. They all seems to do this with time, and can knock like crazy before complete failure.

I probably replaced 8-10 of these during my B20 years, and finally resorted to using the steel B30 gears on the last few.
