loose nuts is correct from using the door. The other tihing I've had happen is the last 4 or 5 teeth at the end of the regular bar were worn down smooth so the regulator couldn't raise the window completely (ie; the teeth were missing).

Boneyard regulators aren't expensive and I tried, unsuccessfully, to make and attach a "replacement teeth" bar to replace the missing regulator ones.
