what is that small pipe threaded into the exhaust manifold?

That's exactly what it was and that's exactly how most of them got "fixed."
Best bet: replace the exhaust manifold with one from a later model without the hole. Split the nuts with a chisel to save the exhaust studs, and the job should go easily. New copper nuts from Volvo should be used too.
