Heater Hose

You need to replace both heater hoses and the heater control valve, along with new hose clamps. Total cost of parts from fcpgroton is probably around $30 USD. Takes about an hour or two, maybe more if your fingers are fat like mine and you have to pull back in the intake manifold to reach the hose that hides under it...
