Have you checked out this Volvo service bulletin? S.B. 23-708R Hard start/stall kit B230 FT w/ AT This S.B. seems to address this issue. The kit consists of an electronic box that modifies the CTS signal in cold weather.

You can read a copy of it by following the link below. After you select the year, make and model, select "Service Bulletins" and then look under "Fuel Sys, Driveability, Filters (Air & Fuel)" Find the service bulletin by its title stated above.

I would also verify that the timing is retarded with a timing light and check the CTS per J.S.'s advice as well.
