Your alternator might have two problems: squashed rubber mounting bushings or a loose belt adjuster. To detect the former, take a long ruler and see if the belt and both pulleys are lined up in parallel. If your alternator has failing bushings, its pulley will not be lined up and will be slightly twisted, usually toward the center of the engine.
The belt adjuster bolt is under the alternator and points parallel toward the engine, parallel to the belt. To tighten the belt, loosen the main alternator fixing nut, then tighten this smaller bolt until you reach the desired tension. Then tighten the main nut and back off this smaller bolt a few turns so it is not under compression (it can break over time). Check your belt to see you get the correct tension.
