There's the bearing, and the seal. Usually if the bearing is going, the seal isn't far behind. But the seal can go independetly of the bearing too. Even if it isn't leaking, grab the fan blades and try to give them a wiggle, see if you can detect any slop in the shaft. You shouldn't feel any. If you do, go ahead and replace it, it will leak very soon.
In terms of difficulty, this is about a 3 on a 1-10 scale (1 being swapping a burnt taillight, 10 being pulling a motor and rebuilding it). No precision work, easy access, very minor disassembly.
